Boy and Man, are both expressions we use when we're going to make a comment that expresses our personal impression of something somewhat emphatically. In other words, nobody is addressing a real "boy" in this statement. Also, this isn't a question, it's an exclamation. We often invert the subject and verb after these words, so this isn't a question (no need for a question mark). It's something like Вот черт, какой же он иногда заноза в заднице.
