It can mean I'M RUNNING A HIGH FEVER, but that's all I can imagine without any further context.

If someone says “I’m running high” they either mean that they have a fever, or figuratively, they’re “overheated” with passion or lust. Sometimes we use this false “I” when we refer to something in close proximity like a car; you’ll hear people say “I pulled into a gas station” when they mean that they pulled THEIR CAR into a gas station. Similarly, sometimes you hear people say they’re running high when they mean their TEMPERATURE/LIBIDO is high.
When you’re on drugs, you say I’M HIGH (no “running” involved)
