Если бы было безуспешно, то, мне кажется, использовался бы модальный глагол. He would insist that he should join the Navy, but in vain.

It can be both: (1) Ever since Adam was a little boy, his father (always) INSISTED on him joining the Navy after graduation. (2) When Adam graduated and was wondering what to do with his life, his father INSISTED that he join the Navy.

I think that "he SHOULD" indicates (1) - настаивал, что он должен пойти . Может быть, он пошёл, а может быть и нет

SHOULD in this case is superfluous, it only indicates that it’s the fathers wish, but isn’t necessary because that’s already inherent in the verb “insist.“ if it was after the fact, it would be SHOULD HAVE.
