Amindjan, that changes everything. If someone’s been sick, or depressed, or stressed out, use ask this in the sense of what they feel like eating because you don’t imagine they have much of an appetite. You should’ve given the context from the beginning.

На самом деле, тут все очень туманно и я понял по-другому.
Вполне возможно, что слова "I am hungry.What are you up for?" принадлежат одному человеку.
Например, один из двоих проголодался и ясно что есть они будут вместе. Поэтому он и спрашивает второго, что/где бы оно хотел съесть или где.

No, in the second scenario, the speaker knows that the listener has been sick and doesn’t know if he has an appetite for anything besides soup. 

BUT it can also be used as in the first scenario, in the sense of “Shall we go eat? What do you feel like eating?“
