You add the second variant as its difficult to combine both in one translation.

I guess my question was which one of those the Russian refers to, if any, since you may not have that distinction in the Russian form of the verb.

Если имеется в виду, что она поняла это в тот момент, на месте, а не заранее, то тогда лучше сказать "я поняла/решила, что мне придется произнести речь".

Вообще же, русское предложение может означать и то, и другое - помощь или необходимость. Но, скорее всего, имеется в виду осознание этого факта заранее.
